Ruth was under care at Huntercombe Hospital, located near Maidenhead, Berkshire, for treatment related to an eating disorder. The unfortunate event that led to her passing unfolded on February 12, 2022, highlighting critical issues regarding patient care and staff inadequacies.
Ruth was supposed to be under constant supervision as part of her treatment to ensure her safety and wellbeing. However, during a critical moment, she was left unaccompanied for approximately 15 minutes, which ultimately led to a tragic consequence. A staff member, who had initially been fraudulently employed and barely trained, was assigned to observe her. This individual, identified as Ebo Acheampong, was found to have entered the hospital using false credentials, a fact which raises severe concerns about the hiring processes at the institution.
Tragically, during a lapse in supervision, Ruth was left to her own devices and managed to go to her room, where she was later found unconscious. Despite efforts to revive her, she succumbed to her injuries two days later at the John Radcliffe Hospital in Oxford. Her death not only shocked her family but also catalyzed scrutiny of the hospital’s operational protocols, particularly in how high-risk patients are monitored and cared for.
During the inquest, it was noted that Huntercombe Hospital had received consistently inadequate ratings from the Care Quality Commission (CQC). It was also discovered that on the day of Ruth’s incident, more than half of the staff members assigned to her ward were absent, exacerbating the vulnerability of the patients under their care.
